The medical history of a 23-year-old asymptomatic male patient with persistent inflammation of unknown origin is presented. Based on an elevated level of the C-reactive protein (CRP) and a lesion in the right iliopsoas muscle on a whole-body dual-modality fluorine-18-2-fluoro-2-deoxy-D-glucose positron emission tomography and computed tomography (FDG-PET/CT), the diagnosis of a sclerosing epithelioid fibrosarcoma (SEF) was made. To the best of our knowledge this is the first reported case of SEF presenting in this way. Elevated levels of the CRP without localizing signs are considered a frequent problem in daily practice. FDG-PET/CT has been applied successfully over the last decade for the imaging of patients presenting an inflammation of unknown origin.
