Abstract:
A classic work up did not disclose the cause of a 3 year recurrent fever in a 45-year-old male. Imaging of the hypothalamic region revealed a suprasellar cyst. After successful transsphenoidal drainage the patient suffered no unexplained fever episodes for more than 15 months. Then the fever episodes returned and a recurrence of the cyst was detected. A second surgical intervention eliminated the symptoms. Imaging of the hypothalamic region is not included in the classic work up for fever of unknown origin. Case reports of a hypothalamic mass causing "fever" are scarce. In the reported case the suprasellar cyst is probably the cause of the recurrent fever.
