Abstract:

The ovarian hyperstimulation syndrome (OHSS) is a frequent complication of assisted reproductive technology. Fortunately, the disease severity is mild or moderate in most cases. The hallmarks of the OHSS are bilateral cystic ovarian enlargement and intravascular/ extravascular fluid imbalance. Moderate or severe OHSS frequently coincides with an early pregnancy. We report the case history of a woman pregnant with twins and a history of moderate OHSS who presented with an adnexal torsion, which was not immediately recognized. This case is described to make clinicians aware of this complication, since the duration of the torsion is a major determinant of later ovarian function. Prompt laparoscopic detorsion is now the treatment of choice, this technique being also recommended during the first trimester of pregnancy.
