Abstract:

Despite the considerable amount of evidence-based studies, several aspects of Lyme-borreliosis remain debatable. In a first part of this manuscript the diagnosis "Lyme-neuroborreliosis" is discussed; indeed, no universally accepted criteria are available. Thus pragmatic criteria are proposed. In a second part, an overview of the treatment of Lyme-borreliosis is presented. Despite the IDSA-guidelines concerning the selection and corresponding dose of antibiotics, evidence-based studies on the duration of the antibiotic treatment remain scarce. Next, pitfalls are discussed concerning the interpretation of serology-tests in patients who suffered from Lyme-borreliosis and were treated following the guidelines. In a last section, the current knowledge of the post-Lyme disease syndrome, including its diagnostic criteria, is reviewed. Finally, it is argued that univocal diagnostic criteria are essential to eliminate the current ambivalence.
