Abstract: A coronary artery fistula is a rare congenital malformation, which can become symptomatic in adulthood. This report describes a 65-year-old patient with a large aneurysmatic fistula who presented with signs of heart failure. Angiographically a large aneurysmatic fistula was found running from the left coronary artery to the junction of the superior vena cava and the right atrium.
